Return and Refund Policy

Billing, Refund, No show, and Discharge from service policies: 

• A $50 fee is expected to be paid in advance of any scheduled appointment and will apply to your balance. 
• We do not accept insurance. Payment is expected in full for weight loss and peptide program prior to ordering medication, and the primary care membership payment schedule is quarterly or paid annually.
• Refunds on medications will not be given once ordered.
• We do not guarantee weight loss results with medication as 7% of the population are considered non-responders.
• Payment for one-time visits is expected prior to your scheduled appointment. 
• Labs and imaging are a separate expense and payment is expected at the time of visit. 
• Refunds for annual or quarterly payments will be returned on a prorated basis.
• Patients who miss more than 2 consecutive appointments will be charged a $10 rescheduling fee, and payment is expected prior to the next appointment. 
• The provider will determine the cause for discharge from our service, and a prorated return will be provided.
